At Evolve Gynecology, we make it easy to request appointments online. Whether you’re scheduling a well woman exam, seeking menopause care, or looking for support with a specific concern, our team is here to help.
Important Policies to protect our patients, staff, and the valuable time of all those involved in your care.
- There is a $25 fee for cancelling or rescheduling within 24 hours of your appointment.
- There is a $50 no-show fee for missed appointments without notice.
- NEW patients who miss their first appointment without notice will not be permitted to reschedule, and ESTABLISHED patients who no-show more than once will be discharged from the practice.
- WE HAVE A ZERO TOLERANCE POLICY AND DO NOT TOLERATE ABUSE OF ANY KIND.
