Evolve Gynecology is transitioning from a traditional insurance based practice to a direct specialty care concierge model beginning April 2026. Our goal is to provide a more personalized experience with improved access and longer visits.

Evolve Gynecology is transitioning away from traditional insurance billing and moving to a concierge model. This allows Dr. Hodges to care for a smaller patient panel and provide a higher level of access, longer visits, and more personalized care.

Insurance-based medicine increasingly limits visit length, access, and care decisions. By stepping away from insurance billing, Evolve can focus on patient-centered gynecologic care built around time, access, continuity, and transparency.

If you are on the fence, we encourage you to review all of your options and contact our Patient Care Coordinator before making a decision. If you are unable to stay, we hate to see you go, but we will ensure your transfer to a new provider for continued care. Contact us through the patient portal to notify our team of your preferred in-network provider so we can send your records and start your transition of care.
Evolve Gynecology will no longer bill insurance for office visits. Patients may continue to use insurance for eligible services such as laboratory testing, medications, outside imaging, and specialist referrals when applicable. We encourage you to inquire with your insurance about requesting reimbursement with your out of network benefits and your HSA or FSA account.

Laboratory testing, medications, outside imaging, and specialist referrals are not included in the membership fee. Insurance may be used when applicable.
Office procedures are not included and are offered on a direct pay basis only. These services are not billed to insurance.
Self pay laboratory discounts are available through Clinical Pathology Laboratories, CPL.
For patients who want primary care access in addition to gynecology, Evolve offers an optional Direct Primary Care add on provided by our Physician Assistant. This service is separate from gynecology membership and has its own scope and pricing.
